Safety reporting
The Air, Maritime and Railway Traffic Accident Investigation Agency uses safety occurrence monitoring mechanisms through the Mandatory Occurrence Reporting system and the Voluntary Reporting system to enhance aviation safety within the European Union (EU) and worldwide.
These systems ensure the reporting, collection, storage, preservation, exchange, dissemination, and analysis of information related to safety in civil aviation.
Reporting of Safety-Related Occurrences is Regulated by:
- Regulation (EU) No 376/2014 of the European Parliament and of the Council of 3 April 2014 on the reporting, analysis and follow-up of occurrences in civil aviation, amending Regulation (EU) No 996/2010 of the European Parliament and of the Council and repealing Directive 2003/42/EC of the European Parliament and of the Council and Commission Regulations (EC) No 1321/2007 and (EC) No 1330/2007;
- Commission Implementing Regulation (EU) 2015/1018 of 29 June 2015 laying down a list classifying occurrences in civil aviation to be mandatorily reported pursuant to Regulation (EU) No 376/2014 of the European Parliament and of the Council; and
- Ordinance on the Implementation of Regulation (EU) No 376/2014 of the European Parliament and of the Council on the reporting, analysis and follow-up of occurrences in civil aviation (Official Gazette Nos. 107/2015, 92/2016 and 28/2019*).*
This Ordinance defines the competent authorities responsible for the implementation of the Regulation in the Republic of Croatia. The Annex to the Ordinance contains applicable occurrence reporting forms and the ICAO risk classification scheme.
Reporting of Safety-Related Occurrences under Articles 4 4. and 5 of Regulation (EU) 376/2014 shall be carried out:
- through the reporting system established by the organisation, in accordance with the requirements set out in Article 7 of Regulation (EU) 376/2014;
if the organisation uses specific reporting software as part of its Safety Management System (SMS), that system must allow direct data transfer to the ECCAIRS software (reports must be compatible with ECCAIRS and ADREP taxonomy, use the standard format, and include all mandatory fields required by Regulation (EU) No 376/2014);
- through the EU reporting portal – ECCAIRS 2, the system for submitting and managing aviation safety occurrences;
- in exceptional circumstances, where neither of the above methods can be used, by submitting a written report using the prescribed form to the following email addresses:
